TC 96-182 is Kentucky's Application for Certificate of Title or Registration. It carries the vehicle description, the odometer statement, the price and trade-in, both parties' details, and a sworn affidavit — and it's filed with the county clerk, who collects the usage tax and enters it all into the state system.
The part that decides the tax is Section 2. Kentucky doesn't charge sales tax on vehicles; it charges a 6% motor vehicle usage tax on the "retail price." On a used vehicle that's the sale price minus the trade-in — but only as attested in an affidavit signed by the buyer and the seller, each notarized. Kentucky titles through the county clerk of the county where the buyer lives — the Transportation Cabinet publishes TC 96-182, but the clerk processes it and collects the 6% motor vehicle usage tax. On a dealer sale the dealer delivers the assigned title to the buyer, or, with the buyer's consent and proof of insurance, files directly with the clerk on the buyer's behalf. The buyer must apply for registration within 15 days of purchase. On a Kentucky-titled vehicle the clerk may not need a separate TC 96-182 (Kentucky says titles issued before 2000 may require one); it is required when the vehicle comes in on an out-of-state title.

Pick the transaction: Duplicate, Title Only, Transfer, First time, Salvage or Classic. A dealer retail sale of a Kentucky-titled vehicle is a Transfer; First time is for a vehicle being titled in Kentucky for the first time — confirm the right type with the clerk if you're unsure. The Duplicate reason boxes (lost, destroyed, damaged, illegible, other) only apply to a replacement title.

Describes the vehicle. The VIN must match the assigned title. Engine type (gas, hybrid, plug-in hybrid, electric) and cylinders are separate boxes; truck weight and motorcycle motor number only apply to those vehicles. If the vehicle is rebuilt, water-damaged, hail-damaged or unrebuildable and the title you're surrendering doesn't show that brand, enter the jurisdiction and title number where the brand was issued.

The tax section. First the odometer reading in whole miles, with a box for mileage that exceeds mechanical limits or isn't actual. Then Date of Sale, Sale Price, Trade-In, Net Cost and Tax — the usage tax is 6% of the net cost. The trade-in only comes off when it's a motor vehicle registered in the buyer's name and presented at the time of purchase; list each trade's year, make, model, VIN and title number on the trade-in lines. Both parties certify the price under penalty of law, and this is the affidavit the clerk relies on — without it (both signatures notarized), the clerk uses book value.

Kentucky's motor vehicle usage tax is 6% of the retail price, collected by the county clerk — there's no separate state or local sales tax on the vehicle. On a used vehicle the retail price is the total consideration minus a trade-in registered to the buyer, as attested in the notarized affidavit, but it can't be less than 50% of the difference between the book trade-in values of the purchased vehicle and the trade. Without a notarized affidavit, the clerk uses book value. Extended warranties and service contracts, loan interest, and rebates applied at purchase are excluded from total consideration. Whether a dealer doc fee is part of total consideration isn't spelled out in the statute — confirm with the Kentucky Department of Revenue. Title fee $9 ($25 speed title) under KRS 186A.130 through 2026; from January 1, 2027 title fees are set by Transportation Cabinet regulation. Registration and lien-statement fees are extra — confirm with the county clerk.

The buyer states whether they have (or have not) applied for a loan on the vehicle, and if not, whether they will within 30 days. On a financed deal, enter the first lienholder's name and address and the county where the lien is filed. Lessee name, FEIN and date of incorporation are for leased vehicles.

Sellers and owners/buyers each sign, and each side has its own attesting official block ("sworn or affirmed before me") with the notary's commission number and expiration. Kentucky defines the total-consideration affidavit as signed by the buyer and the seller with each signature individually notarized. The trade-in credit needs a motor vehicle (not a boat or equipment) registered in the buyer's name and presented at the time of purchase. A vehicle sold before the purchase can't be used as a trade, and credit isn't allowed after the sale.
Kentucky puts a floor on a used-vehicle deal: the taxable price can't be less than half the difference between the book trade-in value of the vehicle sold and the book value of the trade. A paper price below that floor gets taxed at the floor.
A buyer must apply for registration within 15 days of purchase, and the assigned title or bill of sale has to be in the vehicle during that period.
