What the MVT 5-1E is and why it matters

The MVT 5-1E is Alabama's Application for Certificate of Title, the record that puts a vehicle in the buyer's name. On a dealer sale, you as the Designated Agent build it in myDMV, have the buyer sign and date it, give the buyer a signed copy, and upload the application with the surrendered title and supporting documents within 10 calendar days.

Most rejections come from mismatches, not math. ALDOR rejects an application when the owner's name, the co-owner conjunction (AND vs. OR), the lienholder's name, or the odometer reading doesn't agree with the title assignment. The tax side happens on your bill of sale, not at the county: Alabama's state sales tax on vehicles is 2% of the price after a trade-in, plus local rates, and doc fees are taxable. This guide walks the application step by step and flags the checks ALDOR says it rejects for.

There's no blank MVT 5-1 to download and hand-fill. Alabama's title application is the MVT 5-1E, and it's generated inside the Alabama Department of Revenue's online title system (the ALVIN titling system, accessed through myDMV) by a Designated Agent. Every licensed Alabama dealer is a Designated Agent, and ALDOR's rules require Designated Agents to use the online title system to generate and submit title application data. ALDOR's Designated Agent training manual says paper applications stopped being accepted after November 1, 2021. Documents to have before you start

Gather these before you complete the MVT 5-1E. A missing attachment is the number-one reason the Alabama Department of Revenue, Motor Vehicle Division bounces a title application back.

  • The outstanding title (or MSO) assigned to the buyer — Stamp or write "SURRENDERED" on the face before uploading, and keep the original for one year (then electronically for four more).
  • Bill of sale — Must show the buyer, seller, vehicle, date, and signatures, plus the price and sales tax collected, and the notice of the $15 penalty if the vehicle isn't registered within 20 calendar days.
  • Odometer disclosure — Made in the title assignment, or on ALDOR's Odometer Disclosure Statement (MVT 5-5). The mileage on the application must agree exactly with it.
  • Dealer Reassignment Form (MVT 8-3), if the title's assignments are used up — For licensed dealers when every assignment on the title has been completed; it must stay attached to the title it supplements.
  • Power of attorney, if anyone signs for the owner — MVT 5-13 Power of Attorney, or the MVT 8-4 Secure Power of Attorney when a lienholder holds the title or it's lost.
  • Lienholder information, if financed — The lender's exact legal name as it appears in the title or MSO assignment.

Step 1: Start the application in myDMV

Title work lives under Manage Title Applications in myDMV, where you can start, save, edit, void, and rework applications. You must process title work under your own dealer account. ALDOR's rules bar a dealer from processing title applications for another dealer unless both are owned by the same entity. Only Designated Agents can submit the title application package electronically.

Step 2: Vehicle description and odometer

Enter the VIN. myDMV's VIN decoder usually fills in the descriptive data, sometimes only after you pick a trim. If the unladen weight or GVWR doesn't populate, ALDOR suggests looking it up (the NHTSA VIN decoder sometimes has it) and, as a last resort, entering the GVWR in both. The odometer reading has to match the title assignment exactly. On an MSO the reading can be equal to or higher than the assignment. A vehicle last registered in another state must be physically inspected so the VIN plate and description match the application.

Step 3: Owner name(s) and address

Use the buyer's current legal name, identical to the name in the title or MSO assignment, including suffixes like Jr. or III. A variance needs an affidavit explaining it. No more than two owners can be listed. The conjunction matters. "AND" (or no conjunction) means both owners must sign, while "OR" or "AND/OR" means either one can. It has to match the assignment exactly. The owner needs a physical Alabama address. A P.O. box can only be used as the special mailing address, and only when there's no lienholder.

Step 4: Seller and lienholder

The seller information on the application must agree with the supporting documents, meaning your dealership as the seller on a retail sale. If the buyer financed, add the lienholder. ALDOR doesn't require the lienholder in the title assignment, but if it's there, the name must agree exactly. "Bank Loans NA" vs. "Bank Loans" gets the application rejected. If the lender is set up for Electronic Lien and Title, the title is issued as an ELT.

Step 5: Print, sign, and hand the buyer a copy

Print the application and get it signed. The owner(s) must sign and date it. An undated signature is a rejection. With "AND" ownership, both owners sign. With "OR", one is enough. The Designated Agent also signs. Electronic signatures are accepted where ALDOR authorizes them, but not on the physical title or MSO. Give the buyer a signed copy of the application. It works as the permit to operate the vehicle until it's registered, which must happen within 20 days.

Step 6: Assemble, upload, and pay

Build the packet in ALDOR's required order: the signed and dated title application on top, then the ownership document (MSO or title, stamped SURRENDERED), then everything else (reassignment forms, bill of sale, power of attorney). Mark the application complete when you upload, or it won't go to ALDOR for review. Upload within 10 calendar days of creating the application, and pay the $15 title application fee plus the $1.50 Designated Agent commission. ALDOR's training manual says payment is due within 10 days of completion and that you'll be locked out of the system if an application isn't submitted within 30 days. Putting it on hold adds 30 days. Resubmitting a rejected application is free.

Common MVT 5-1E mistakes that cause rejections

Owner name that doesn't match the assignment

ALDOR rejects an application when the owner's name differs from the title or MSO assignment, even "John Allen Smith" vs. "John A Smith" or a missing suffix. Enter the name exactly as assigned, or include an affidavit explaining the variance.

AND/OR that doesn't match (or a missing co-owner signature)

The conjunction joining two owners must match the assignment exactly. With AND (or no conjunction), both owners sign. With OR, one can. A mismatch or a missing required signature is a rejection.

Lienholder name that's close but not exact

If the assignment shows the lienholder, the application must match it exactly. ALDOR's own example is that "Bank Loans NA" vs. "Bank Loans" is rejected. Copy the lender's name from the assignment rather than typing it from memory.

Signed but not dated

The owner(s), or an authorized representative for a business, must sign and date the application. ALDOR rejects an undated application, so check the date line before you upload.

Uploading late or leaving the tag clock running

Upload the packet within 10 calendar days of creating the application, and make sure the buyer registers within 20 calendar days of the sale. Alabama law requires your bill of sale to warn of a $15 penalty for missing that 20-day window.

The MVT 5-1E, Application for Certificate of Title.
