Form 701-6 is Oklahoma's Application for Certificate of Title. It covers the vehicle, the owner, any lien, and a sworn oath with a notary block. Page 2 has the used tire recycling fee count, the damage, theft and flood disclosures, and a VIN/odometer inspection block. Notice there's no price or tax line. The purchase price comes from the purchase agreement, and the licensed operator figures the tax from it and stamps the form.
Oklahoma's quirk is that a vehicle purchase carries two separate taxes. The motor vehicle excise tax on a used vehicle is $20 on the first $1,500 of value plus 3.25% of the rest. On top of that there's a 1.25% state sales tax, with no city or county add-on. The two use different bases: by statute the sales tax is figured on the price after the trade-in, while the excise tax is figured on the actual sales price before any trade-in credit. This guide walks the current (07/2025) form in the order it prints.
Form 701-6 is issued by Service Oklahoma, which took over motor vehicle services from the Oklahoma Tax Commission. Titles are processed at Service Oklahoma or at a licensed operator (the former tag agencies). On a dealer sale, the dealer submits the pre-registration within two business days and issues a 10-day temporary tag. The buyer then has two months from the purchase date to get the title, pay the sales and excise tax, and fully register the vehicle.
Gather these before you complete the Form 701-6. A missing attachment is the number-one reason the Service Oklahoma (licensed operators process titles) bounces a title application back.
The top block identifies the vehicle. The identification number must match the title or the Electronic Title Bill of Sale. Pick the color from the list printed on the form. Oklahoma plates belong to the owner, not the vehicle, so if the buyer is keeping a plate from their previous vehicle, enter it on the transfer line. Next, choose a paper or electronic title. Electronic is the default when there's a lien, and a paper title can be requested when there isn't one. The entry-date and employment-date lines only apply to vehicles brought in from another state, so leave them blank on an in-state dealer sale. The "Place Motor Vehicle Tax Stamp Here" box is reserved for the agency.

The buyer, meaning the person or business the title will be issued to. Enter the owner name, address, daytime phone and email, plus the owner's driver license number or, if the owner isn't an individual, the business's Federal Employer Identification Number.

If the buyer financed, list the lienholder here. The form notes that when a vehicle is subject to a lien, the lienholder holds the title. Enter the lien date and the Lienholder ID, which Service Oklahoma assigns to Oklahoma lienholders.

The owner, or the owner's legal agent, swears under penalty of perjury that they own the vehicle described and that the statements on the form are true. The oath is set up as a notarized jurat, so the notary completes the state, county, date, commission expiration, signature and seal. Sign in front of the notary, not before.

Oklahoma charges a used tire recycling fee when a vehicle is first titled or registered in the state. Enter the number of tires for each rim size, including the spare. There's a line for automobiles with rims of 19.5" or less, one for motorcycles, and one for other vehicles with rims over 19.5". Oklahoma law also requires the tire count and rim diameters on the dealer's bill of sale, so the two should match. Separate boxes cover IRP-registered vehicles and commercial trailers.

For a vehicle 10 years old or newer, the owner or legal agent answers whether it has been damaged by collision or other occurrence, and whether it was recovered from a theft. The exception is when a branded title from another state is being surrendered. A yes answer means stating the repair cost as a percentage of fair market value. The flood disclosure applies to vehicles of any age. If there was water or flood damage, the owner then answers whether it meets the statutory definition of a flood-damaged vehicle (47 O.S. § 1105). A VIN and odometer check. The inspector physically confirms the VIN and the odometer reading and certifies them with their title or position, city, state and, if applicable, licensed operator number. It mainly comes up with vehicles arriving on an out-of-state title. Service Oklahoma lists a $4 VIN inspection fee. Ask the licensed operator whether your transaction needs one.

By statute (68 O.S. § 1355), the 1.25% sales tax is figured on the difference between the sales price and the trade-in value. The excise tax is based on the actual sales price before any discounts or credits for a trade-in (68 O.S. § 2104, as amended by HB 1183 effective July 1, 2026), so the trade doesn't reduce it. Keep the two separate on the deal recap.
A used vehicle is taxed $20 on the first $1,500 of value plus 3.25% of the remainder. A new vehicle is taxed 3.25% of its value. A unit being titled off an MSO that hasn't been titled and taxed in Oklahoma before is generally charged the new-vehicle rate; confirm edge cases with the licensed operator.
Under the Mason Treat Act, the dealer must submit pre-registration within two business days of the sale and issue the 10-day temporary tag. The buyer has to have the metal plate and pre-registration decals on within ten days. Build it into your delivery checklist.
Service Oklahoma gives a newly purchased vehicle two months to title and register without penalty charges, though its FAQ also says penalties on a newly purchased vehicle accrue from the 31st day after ownership assignment, so don't plan on using the whole window. Most vehicles pay $1 a day in penalties (up to $100), and by law those can't be waived. Get the buyer a complete package so the licensed operator visit is one trip.
The damage/theft declaration is required for vehicles 10 years old or newer, and the flood disclosure is required for every vehicle regardless of age. Leaving them blank stalls the title. Answer them from what you actually know about the unit's history.

For a used vehicle, Oklahoma's motor vehicle excise tax is $20 on the first $1,500 of value plus 3.25% of the value above $1,500 (68 O.S. § 2103). Value is the actual sales price. On a $15,000 used car that works out to $20 + 3.25% × $13,500 = $458.75, before the separate 1.25% sales tax.
Yes, 1.25% state sales tax on top of the motor vehicle excise tax. City and county sales taxes don't apply to motor vehicle sales. Both taxes are paid when the vehicle is titled.
For the 1.25% sales tax, yes. Under the 2023 amendment to 68 O.S. § 1355, it's figured on the difference between the sales price and the trade-in value. For the excise tax, no. Since July 1, 2026 (HB 1183), the excise value is the actual sales price before any discounts or credits for a trade-in (68 O.S. § 2104).
Service Oklahoma lists the vehicle title fee at $11. Registration is extra. A standard vehicle is $96 a year for years 1-4, stepping down to $26 at 17+ years. Other fees, such as the used tire recycling fee and a VIN inspection fee, may apply depending on the vehicle.
