The MV-21-A is one car's paperwork. A 20-second read on your lot, and the tax-and-fee math for the whole deal — built for dealers, no signup. Free calculators dealers actually use — no signup, no email.

The MV-21-A is how a lender puts a lien on an Oklahoma title. If you run buy-here-pay-here, that lender is you. Every car you finance in-house gets a lien entry form filed with the title paperwork, and the paperwork you get back from that filing is what you use to release the lien when the customer pays off.
So this form matters twice: at the sale, when you perfect the lien, and at payoff, when Service Oklahoma's guidelines give you seven business days to get a signed release to the customer and to Service Oklahoma. This guide walks the MV-21-A box by box from the dealer's seat, then covers the 25-day perfection window, the fees, and the release.
The current revision is SOK Form MV-21-A, Revised 12/2025. It records (perfects) a lien on an Oklahoma vehicle, boat, or outboard motor. Oklahoma has no separate lienholder release form. When the MV-21-A is filed, the lender gets back a lien release form and a lien fee receipt, and those are what you sign at payoff. The form itself says to type and print three identical copies; Service Oklahoma's FL-21 guidelines (Revised 2/2025) say four. Ask your tag agent which count they want.
The lien entry form travels with the title paperwork to Service Oklahoma or a licensed operator (tag agent). Have these with it.
The top block is the debtor: name(s), last name first, then address, city, state, and ZIP. On a BHPH deal that's your buyer and any co-buyer, named the same way as on the title application. Use one lien entry form per vehicle; the form says so.

Secured Party is your dealership: name, Lienholder ID, address, city, state, ZIP. The right side, Assignee of Secured Party, is for when the lien is assigned to someone else, such as a finance company that buys the contract. If you're carrying the note yourself, leave the assignee side blank.

The vehicle line is year, make, body type, and VIN/HIN. FL-21 is blunt about the VIN: it's the sole source for locating a perfected lien in Service Oklahoma's files, an error jeopardizes perfection, and errors sometimes start on the MSO, so check it against the vehicle yourself. Date of Security Agreement is the date your buyer signed the contract. Original Oklahoma Title Number applies when the vehicle already has an Oklahoma title.

Sign on Secured Party/Assignee Signature and fill in Date Executed. FL-21 says that for perfection to date from the form's execution, the MV-21-A and all required documents and fees must reach Service Oklahoma or a licensed operator within 25 days after the security agreement is executed. Later than that, perfection happens on delivery. The operator stamps the date, time, and receipt number on the title paperwork, attaches a copy of the MV-21-A and the lien fee receipt, and returns to you a copy of the MV-21-A, a copy of the lien fee receipt, and a four-part lien release form. Keep those with the deal. They're your release paperwork at payoff.

Service Oklahoma's FL-705 creditor letter says that as of July 1, 2022, Oklahoma is a title-holding state: once the title is issued, it's mailed to or held electronically for the lienholder. On a BHPH sale, expect the title to come to your dealership, not the buyer. FL-21 also says the lender mails the title documentation to the debtor with a copy of the MV-21-A or a notice that the debtor must register the vehicle and pay all fees and taxes within 30 days of purchase. For a lien on an electronic Oklahoma title, Service Oklahoma uses the SOK 901 Notice of Security Interest instead of the MV-21-A.

FL-21 says that within seven business days after the debt is satisfied, the lender must furnish a signed and dated copy of the lien release to Service Oklahoma and to the debtor, and failure makes the lender liable to the debtor for a $100.00 fine plus any other losses the failure causes. The FL-705 letter adds that the title goes to the debtor with the release. OAC 670:20-41-6 lists what releases a perfected lien: the signed and dated release copy of the lien entry form, or the signed and dated lien release receipt from when the lien was perfected. If you have neither, a typed, notarized release statement from the secured party, showing the secured party's name, the debtor's name, and the VIN, with a representative's notarized signature. The owner then gets a new title without the lien on proof of release and the $11.00 title fee plus $1.73 mail fee.

FL-21 says the VIN is the only way Service Oklahoma finds a perfected lien, and an inaccurate VIN jeopardizes perfection. Check it against the car, not just the paperwork.
Perfection then dates from delivery, not from the form's execution. File with the title paperwork right after the sale.
FL-21 says MV-21-A forms must be typed, computer generated, or legibly printed. Illegible forms are rejected.
It's the easiest way to release at payoff. Without it or the lien release receipt, you're down to a typed, notarized release statement under OAC 670:20-41-6.
FL-21 says the signed, dated release goes to both Service Oklahoma and the debtor within seven business days.
The form says to use one Lien Entry Form for each vehicle, boat, or outboard motor.
It's Service Oklahoma Form MV-21-A, Lien Entry Form, Revised 12/2025. The lender fills in the debtor, the secured party (and assignee, if any), the vehicle, the security agreement date, and signs it. It's filed with the title paperwork at Service Oklahoma or a licensed operator to perfect the lien.
The MV-21-A lists, for vehicles, a $10.00 lien entry fee, an $11.00 title fee, and a $1.73 mail fee. A lien-only filing is the $10.00 lien fee and the $1.73 mail fee.
FL-21 says that for perfection to date from the form's execution, the MV-21-A and all required documents and fees must be delivered within 25 days after the security agreement is executed. After that, perfection happens when it's delivered.
Not a separate one. When the lien is filed, the lender gets back a lien release form and a lien fee receipt. At payoff you sign and date the release and send it to Service Oklahoma and the debtor. OAC 670:20-41-6 also accepts the signed lien release receipt, or a typed, notarized release statement if you have neither.
Seven business days after the debt is satisfied, per Service Oklahoma's FL-21 guidelines. A lender that doesn't furnish the release is liable to the debtor for a $100.00 fine and any other losses caused by the failure.
Only as the fallback. OAC 670:20-41-6 accepts a typed release statement from the secured party when you don't have the lien release form or receipt, and it has to be notarized and show the secured party's name, the debtor's name, and the VIN. DealerVLO's BHPH servicing prints a release letter on your letterhead with the owners, vehicle, and VIN, but it isn't a substitute for the release paperwork you got at filing, and it would still need a notary.
