Form 411007 is Iowa's Application for Certificate of Title and/or Registration — the one form that puts a vehicle in the buyer's name and registers it. On a dealer sale, the buyer fills in and signs the owner side, the dealer completes the "For Dealer Use Only" price box, and the dealer forwards the application, the assigned title, and the fees to the county treasurer within 30 calendar days of delivering the vehicle.
The number that matters is the fee for new registration. Iowa doesn't charge sales tax on vehicles subject to registration; instead it charges a one-time fee for new registration of $10 plus 5% of the purchase price — the $10 base was added January 1, 2025, so older worksheets undercharge. A qualifying trade-in comes off the price first, and dealer doc fees aren't part of the price the 5% is figured on. This guide walks the current 411007 (01-25) box by box, in the order it's printed.
Form 411007 is issued by the Iowa DOT but filed with a county treasurer — Iowa titles and registers vehicles through the 99 county treasurer offices, not a state DMV counter. The Iowa DOT publishes it as an online fillable form (it prints to a three-page PDF; pages 2 and 3 are supplemental and aren't submitted if blank). On a dealer sale, the buyer is the applicant and signs, and the dealer completes the "For Dealer Use Only" box, collects the fees, and must forward the application to the county treasurer within 30 calendar days of delivery. A dealer can also submit it through Iowa's Electronic Registration and Title (ERT) system — there's a checkbox on the form authorizing exactly that. The current revision is 411007 (01-25).
Gather these before you complete the Form 411007. A missing attachment is the number-one reason the Iowa Department of Transportation (filed with your county treasurer) bounces a title application back.
The header is quick: check Regular Title (or Salvage Title), and enter your D/R number — your Iowa dealer number — in the top right. The Registration Month line is also up there. Then the Owner Information block: the application is made to the county treasurer designated by the owner, so the buyer's bona fide residence address — including the county — drives where it's filed. Up to three owners can be listed; for two, check OR or AND for the ownership status. An individual gives a birth date and an Iowa DL/ID or Social Security number; a business gives its FEIN instead. The three checkboxes at the bottom of the block send you to page 2 (primary user, for a non-resident or business owner) or page 3 (a third owner, or a one-time mailing address).

Describes the vehicle and any trade. The VIN must match the assigned title and the vehicle. Iowa Code 321.20 asks for make, model, body type, cylinders, and fuel where they apply; tonnage, GVWR, and square footage only apply to trucks, trailers, and the like. If the buyer is moving a plate they already own onto this vehicle, enter it and the plate type. The trade-in VIN lines matter for the fee: they document the trade that reduces the price in the Purchase Price section (more than two trades go on page 3). The purchase date starts the clock — the form itself warns that the Iowa title must be obtained within 30 days of purchase or penalties may apply.

If the buyer financed, record the lien here so it's noted on the new title. Check how many security interest holders there are (None, One, Two, Three), then give the first lienholder's name and full address plus either its FEIN or SSN, or — by checking the box — its Electronic Lien and Title (ELT) identifier. Second and third liens go on page 3. Iowa charges $20 for each security interest noted. The escrow-lien checkbox is only for a lien already submitted to the county as an escrow lien.

The left side of the Purchase Price section is the buyer's: the purchase price less any trade, which is what the fee for new registration is figured on. The two checkboxes are for an owner claiming an exemption from the fee (with its code from page 2) or a business trade exemption for a truck — leave both blank on an ordinary retail sale. Then every owner signs and dates, certifying under penalty of perjury that the application is true and correct. A firm, association, corporation, or attorney in fact signs on the "By" line.

This box is where the dealer certifies the price. Start with the sale price (the total consideration, including freight, accessories, and services), subtract the trade-in, subtract charges exempt from the fee for new registration, and subtract any manufacturer rebate applied to the price. What's left is the fee for new registration price, and the fee is $10 plus 5% of that. Two rules decide the trade line: the traded vehicle has to be the kind you normally sell, and the owner on the trade's title has to be the same as the owner on the new title (Iowa allows limited exceptions for spouses, lineal family members, and cosigners). Iowa's rules exclude dealer doc fees from the purchase price, so a doc fee included in the sale price comes back out on the exempt-charges line rather than being charged the 5%. Then record the date you issued the registration applied for card (or write "none"), the registration fee collected, and sign with your dealer number and dealership name.

Iowa charges a one-time fee for new registration of $10 plus 5% of the purchase price instead of sales tax; it's set by state law, so it doesn't vary by county. A trade-in comes off the price only if it's a vehicle type the dealer normally sells and it's titled to the same owner as the new vehicle. Dealer doc fees are excluded from the purchase price (don't add them here). Title fee is $30 plus a $5 surcharge; add $20 for each lien noted. The trade-in only reduces the price when the traded vehicle is a type you normally sell and the owner on its title matches the owner of the new vehicle (with limited exceptions for spouses, lineal family members, and cosigners). A boat traded on a car, or a trade titled to someone who isn't on the new title, doesn't count. The fee is then due on the full price.
Iowa's Department of Revenue rules exclude dealer document-processing fees from the purchase price for the fee for new registration. Show it as a charge exempt from the fee in the dealer box.
