Form 3019 is Missouri's Odometer Disclosure Statement: a separate mileage statement for when the title can't carry one. Its own instructions put the rule plainly. When a title or manufacturer's statement of origin doesn't meet the federal requirements for a proper odometer disclosure, the seller issues a separate statement and attaches it to the ownership document at the transfer. And if the Missouri title was issued after August 28, 1989, the disclosure goes on the back of the title, not on a 3019.
So most Missouri retail deals never need one. Dealers run into it on older titles and out-of-state titles without a conforming odometer section, and on mileage corrections. This guide walks the top block and one disclosure space box by box, covers the discrepancy statement and the correction rules printed on the form, and the five-year record rule.
The current revision is Form 3019 (Rev. 01-2021), a two-page PDF from the Missouri Department of Revenue: the form on page one, instructions on page two. Page one has four disclosure spaces, one per transfer. The PDF is fillable, so you can upload it to DealerVLO once under Settings → Forms. Its fields have generic names ("Text1", "Text9"), but DealerVLO recognizes this form and suggests a mapping for the first space: the vehicle block, purchaser name, seller name, dealer number, reading, and the two mileage boxes. Map the date and the two address boxes yourself, and check each suggestion once against this guide. Leave the hand-printed name boxes (the form requires those by hand) and spaces two to four unmapped. The signature lines aren't fields; they're signed in ink.
Gather these before you complete the Form 3019. A missing attachment is the number-one reason the the Missouri Department of Revenue, Motor Vehicle Bureau bounces a title application back.
Federal rules put the disclosure on the title (49 CFR 580.5(c)), and Form 3019's instructions say the same for Missouri titles issued after August 28, 1989. The 3019 is for a title or MSO that doesn't meet the federal requirements for a proper disclosure: in practice an older title, or an out-of-state title, with no conforming odometer section. The DOR's titling manual also asks for a new odometer disclosure statement, completed in full by buyer and seller, when someone applies for a corrected title because of the mileage. When the title isn't available at the sale because the lender holds it or a duplicate is needed, Missouri dealers use the Secure Power of Attorney (Form 5086), not a 3019. And for an extra dealer reassignment, the DOR's dealer manual describes the secure Reassignment of Ownership by Registered Dealer (Form 2447), which gives a registered Missouri dealer one additional assignment as an extension of the ownership document.
| Field | Who fills it |
|---|---|
Missouri title issued after August 28, 1989 Disclose on the back of the title. No 3019. | You (dealer) |
Title or MSO without a proper federal disclosure section The seller completes a 3019 and attaches it. | You (dealer) |
Corrected title because of a mileage error New disclosure statement plus a signed explanation. | You (dealer) |
Title held by the lender, or a duplicate title needed Secure Power of Attorney, Form 5086. Not for floor-plan titles. | You (dealer) |
The top of the form describes the vehicle once for all four spaces: year, make, VIN, title number, the state that issued the title, model and body style. Copy them from the title. In the PDF these are "Text1" through "Text7", left to right, top row first.
| Field | Who fills it |
|---|---|
Year PDF field "Text1". | You (dealer) |
Make PDF field "Text2". | You (dealer) |
Vehicle identification number PDF field "Text3". | You (dealer) |

State PDF field "Text5". The state that issued the title. | You (dealer) |
Model / Body style PDF fields "Text6" and "Text7". | You (dealer) |
Each of the four spaces is one transfer. Use the first open space, and map only that space's fields in DealerVLO. The purchaser's name and address, the date, then the seller's (firm) name, dealer number and address. The form says that if the purchaser or seller is an agent or officer of a firm, record the official position after the printed name. Each address is one box, and DealerVLO fills a field with one value, so map the street address and add city, state and ZIP by hand, or type the whole line.
| Field | Who fills it |
|---|---|
Purchaser(s) name (printed or typed) PDF field "Text9". From the deal. | You (dealer) |
Date PDF field "Text10". The sale date. | You (dealer) |
Address (purchaser) PDF field "Text11". One box for the full address. | You (dealer) |
Seller(s) (firm) name (printed or typed) PDF field "Text12". Your dealership on a retail sale. | You (dealer) |
Dealer no. PDF field "Text13". Your dealer license number. | You (dealer) |
Address (seller) PDF field "Text14". | You (dealer) |
Enter the exact reading with no tenths. The seller states that the odometer reads that number and, to the best of their knowledge, reflects the actual mileage unless a box is checked. There is no box for actual mileage. The DOR's titling manual says that when the mileage is actual the seller should not check any of the boxes, and the new title will say ACTUAL MILEAGE. "Mileage in excess of its mechanical limits" is for a rolled-over odometer: the form's example is a car with 125,000 actual miles showing 25,000, where you enter 25,000 and check that box. "Mileage reading not actual" is for when the true mileage is different or unknown, and the seller must attach a statement explaining all the facts. In the state's PDF the two boxes in each space are one field ("Check Box1.0" in the first space). DealerVLO's mapping screen shows each box as its own row: box "Yes" is in excess, box "b" is not actual. Its suggestion maps both to the deal's odometer status, so the right box gets ticked and both stay blank when the mileage is actual.
| Field | Who fills it |
|---|---|
Odometer reading (no tenths) PDF field "Text15". Read it at the time of transfer. | You (dealer) |
Actual mileage No box. Leave both unchecked. | You (dealer) |
Mileage in excess of its mechanical limits Box "Yes" of "Check Box1.0". Shares a field with the box below. | You (dealer) |
Mileage reading not actual (warning, odometer discrepancy) Box "b" of "Check Box1.0". Attach the seller's statement of facts. | You (dealer) |
Below the reading are the purchaser's and seller's signatures, then the hand-printed names. The instructions on page two say everything on the form can be typed or computer generated except the printed names and signatures, and that one purchaser and one seller must sign and hand-print their names. Leave the hand-printed name fields ("Text37" and "Text38" in the first space) unmapped. The DOR's titling manual says odometer disclosure statements no longer need a notary for transfers on or after August 28, 1989.
| Field | Who fills it |

Page two covers mistakes. If an error happens in the second, third or fourth space, draw a large X through that space, make the correct disclosure in the next space, and have the purchaser and seller complete and sign a statement explaining the error, attached to the 3019. Both documents go with the title application. An error in the fourth space means a new 3019 attached to the original, with the explanation. The form also says the purchaser and seller should each keep a copy. The DOR's dealer manual (revised January 2026) requires dealers to keep clear and complete copies of all odometer disclosure documents issued and received for five years, which matches 49 CFR 580.8. Under section 407.536, RSMo, falsifying an odometer reading on these documents is a class E felony.

The form says a Missouri title issued after August 28, 1989 takes the disclosure on the back of the title. Use the 3019 only when the title or MSO can't carry a proper disclosure.
There isn't one. When the reading is actual, leave both boxes unchecked.
The form allows everything to be typed except the printed names and signatures. Leave those fields unmapped in DealerVLO.
In the state's PDF both statements in a space are one field. Map it box by box: "Yes" to exceeds mechanical limits, "b" to not actual. DealerVLO's suggestion does this for the first space.
The form says the seller must attach a statement explaining all facts regarding the discrepancy.
The DOR's titling manual says the Secure Power of Attorney (Form 5086) is for a title held by the lienholder or one that needs a duplicate. It can't be used for a floor-plan title.
The Missouri Department of Revenue publishes it as Form 3019, a free fillable PDF on dor.mo.gov. The current revision is 01-2021. The download button at the top of this guide links straight to it.
No. The form's instructions say that for a Missouri title issued after August 28, 1989, the disclosure is completed on the back of the title rather than on a separate statement. The 3019 is for a title or MSO that doesn't meet the federal requirements for a proper disclosure.
No. The DOR's titling manual says Missouri odometer disclosure statements no longer need a notary for transfers on or after August 28, 1989.
The form lists model year 2010 or older and at least ten years old at the transfer; model year 2011 or newer and at least twenty years old; vehicles over 16,000 pounds GVWR; and new vehicles transferred from one franchise dealer to another. The federal rule at 49 CFR 580.17 also exempts vehicles that aren't self-propelled.
Draw a large X through the space, make the correct disclosure in the next space, and attach a statement signed by the purchaser and seller explaining the error. If the mistake is in the fourth space, attach a new 3019 to the original with the explanation. Both documents go with the title application.
Five years. The DOR's dealer manual requires clear and complete copies of all odometer disclosure documents issued and received, and 49 CFR 580.8 sets the same five-year rule.
