ITD 3337 is Idaho's Application for Certificate of Title — the form that puts a vehicle in an Idaho resident's name when the deal isn't written on an Idaho dealer's controlled ITD 0502. That makes it the form an out-of-state dealer fills out for an Idaho buyer, and the one Idaho lenders and manufactured-home dealers use. It's filed with a county assessor's motor vehicle office, and whoever files it has 30 days from delivery.
The section that decides whether it goes through is Section 3, Sale Information. Idaho taxes at a flat 6%, the doc fee is taxable, and a trade-in lowers the taxable price only on a dealer sale. The sale box also asks whether you collected Idaho tax under an Idaho seller's permit or left it for the buyer to pay at the county. Know which Idaho form you actually need. ITD 3337 is the public Application for Certificate of Title, and page 2 of the form says it's intended for Idaho financial agencies, licensed Idaho manufactured-home dealers, out-of-state dealerships, and private-party sales to Idaho residents conducted outside Idaho. A licensed Idaho retail vehicle dealer normally titles its sales on ITD 0502, the Report of Sale and Application for Certificate of Title — a serial-numbered form dealers order from ITD (on ITD 3195) and must use in control-number order, so it isn't a download. Gather these before you complete the ITD 3337. A missing attachment is the number-one reason the Idaho Transportation Department (ITD) — Division of Motor Vehicles bounces a title application back.
Up to three owners, each with a full legal name (or business name), an Idaho driver's license number (or SSN, or EIN for a business), sex, and date of birth. The Or / And boxes set how the title can later be signed off: OR lets any owner sign; AND requires every owner. LSR / LSE mark a lessor and lessee, and DBA puts a business name on its own owner line. An applicant with no Idaho license or ID needs the Verified Photo ID box checked. Below that, the owner's Idaho physical address, plus a mailing address if it's different.

The unit itself. The VIN must match the title and the vehicle. Most of the second and third rows (wheel base, length, width, hull material, horsepower, propulsion) apply mainly to trailers, ATVs/UTVs, motorhomes, and boats; fill in fuel type and weight where they apply to the vehicle. The tax section. Start with the Idaho seller's permit number (the form label says it's required for leasing and rental companies, and it's how the county knows Idaho tax was collected and remitted) and the purchase date. Check Lease, Rental, Tax Not Included, or Tax Exempt Sale as they apply. Then the math: Gross Sales Price, with the doc fee in it (Idaho taxes dealer doc fees). Under Dealer Sales Additional Information, subtract qualifying rebates and the trade-in allowance to get the Adjusted Gross Sales Price, and 6% of that is the Net Idaho Sales Tax Due. Net Idaho Sales Tax Collected is what you actually took in at the sale. An out-of-state dealer that doesn't collect Idaho tax checks Tax Not Included, and the buyer pays at the county. Credit is given for sales tax the buyer paid to your state.

For a vehicle coming off another state's title: the previous state, the previous title number, and any brand that state carried — Rebuilt Salvage, Repaired, Reconstruct, or Other (write it in, e.g. recovered theft). Brands carry forward, so copy them off the surrendered title.

Your dealership or lender, as it appears on your license: name, the printed name and job title of the person signing, address, phone, fax, and email. The checkbox certifies you're an Idaho-registered financial institution or an out-of-state dealership authorized by Idaho, that you physically inspected the vehicle, and that the VIN on the form is correct. The authorized signature then certifies the application, confirms you witnessed the applicant's signature, and releases your interest in the vehicle unless you're listed as lienholder or owner. Finish with the date and the city of the county office you're submitting to.

The applicant signs, certifying under penalty of perjury that they own the vehicle and it won't be subject to a lien before the title arrives unless one is listed. Include an email and daytime phone number, and date it. On an AND title, every owner signs. Your authorized representative has to witness these signatures or the county won't process it.
